Not necessarily, it depends on the situation. It can be an endearment or it can be rude, depends how you use it and who you say it to..

Okay, so what Happymurtard said is true even though he said he is being fictitious. Also even though you use the term fuck that I feel is derogatory and never used for kind relationships you could use it endearingly. So my first thought after reading both comments on MILF was correct context and culture must be facts in considering it's meaning.
